Any child in Steuben County
(0-21 years of age) who has or
is suspected of having a serious or chronic physical condition and who
also
requires health and related services of a type or amount beyond that
required
by children generally is eligible for services through the Steuben
County
Physically Handicapped Children's Program.
<>The Physically
Handicapped Children's Program covers more than 150 physically
disabling
conditions and chronic illnesses - including cerebral palsy, epilepsy,
cystic
fibrosis, diabetes, blood dyscrasias, congenital malformations, cancer,
hearing
disabilities, traumatic deformities, hernia, prematurity and
orthodontia.
After receiving a
referral, the Physically Handicapped Children's Program contacts the
physician
for specific recommendations. There are
two parts to the Physically Handicapped Children's Program. The DIAGNOSIS AND EVALUATION part of
the program is available without financial eligibility requirements. Our Program will arrange appointments and/or
testing with New York State approved physicians and/or specialty
centers for
diagnostic evaluations. The
specialist's appointment, x-rays, lab work, EKG's, etc. are paid by NYS
funds
after private medical insurance is applied. The
second part of the Program is when a child has already
been
diagnosed and a plan of treatment determined. The
TREATMENT part of our Program requires
financial eligibility
- either by applying through Medicaid or applying through PHCP's
confidential
financial investigation. Services
authorized are paid by State and County governments.
We do not open cases where recommended services cost less
that $200.00,
as there is a $200.00 deductible per authorization for eligible
families.
In addition to financial
help, PHCP provides assistance in
obtaining health services and community services. The
Program will refer families to services and advocate on their
behalf.
